80 Background From 2016 to 2022, Non-Hispanic Black infants were at 2 times more likely to die than Non-Hispanic White infants in Butler County.
The Program Specialist works collectively with the maternal and child health team to reduce racial inequities in birth outcomes and prioritize the population most at risk for poor birth outcomes.
Grant-funded programs include the Ohio Equity Initiative and Cribs for Kids.
Essential Functions and Goals Under the general direction of Health Promotion Director the Program Specialist identifies and engages with people who are pregnant in order to connect them to clinical care and/or social services.
Reviews access to services by acting as a connection between medical care, social services, and the community.
The overall objectives of this position is to improve birth outcomes, increase the number of families served by home visiting programs, and provide support.
Additionally, this position will coordinate with partners and the community to ensure families understand and practice infant safe sleep.
Minimum Qualifications High School Diploma or its equivalent.
Basic computer skills.
Passionate about improving maternal and infant health.
Strong interpersonal and communication skills and the ability to work effectively with a diverse community are essential.
o Interpersonal skills include professionalism, sensitivity, empathy, flexibility, accessibility, patience, resourcefulness, enthusiasm, and the ability to be non-judgmental.
Exhibit excellent customer service.
This position requires the candidate to have a reliable personal vehicle, and a valid Ohio driver's license in good standing and insurable by the Board of Health's Policy.
Qualifications Preferred Successful completion of a Certified Community Health Worker formal training program, or health/social service related degree.
Butler County resident or knowledgeable of Butler County resources.
Knowledge of social determinants of health and health disparities.
Experience working in a community based setting for at least 1-2 years.
Bilingual or multilingual (French or Spanish) helpful.
Primary Duties Include (not limited to) Traditional and non-traditional community outreach o Responsible for establishing trusting relationships with people who are pregnant and/or families, while providing general support and encouragement.
o Maintain or develop personal knowledge about community and social resources.
o Maintain a portfolio of local referral resources to services, evidence-based interventions, clinical and social services in order to connect families to appropriate services.
Participant assessments o Complete telephone or one-on-one interviews with people who are pregnant through the use of the Ohio Department of Health screening tool.
o Provide follow-up to program participants via phone calls and other means as necessary to ensure their needs are met.
o Document referrals and address risk status via the Ohio Dept.
of Health identified platform.
Community coalitions o Attend regular staff meetings, trainings and other meetings as requested.
o Collaborate to review documented barriers and advocate the challenges experienced by program participants.
Cribs for Kids, infant safe sleep o Develop and maintain community partnerships in order to meet monthly goals o Complete and/or contribute on program/progress reports.
o Maintain and track the inventory of program materials including but not limited to, Cribette's (e.
g.
Pack-n-Play), educational tools, and program office supplies.
Time management skills o Includes accountability for work done in and outside the office, as well as accurate entry into time accounting database.
